1. Fill out and submit the online form below.  Once our staff has recieved your information we will contact you to confirm an appointment. (scroll down and start filling the form out now)
2. Download the PDF Forms from the following link, print, complete and call our office at 803-996-2020 to schedule an appointment.  You can bring the forms in with you during your initial visit.

3. Simply call us at 803-996-2020 and schedule your appointment.  You may fill the forms out while you wait.
